Improve patient-population health by providing integrated care services in concert with Primary Care, remotely, and to as broad a segment of the society as possible. Maintain fidelity to the Integrated Care model while providing evidence based and compassionate care.
Vision
1. Behavioral health services are essential healthcare services;
2. The integrated care model, in partnership with Primary Care, can help achieve the Institute of Healthcare Improvement’s Triple Aim: A) Better Patient Experience of Care; B) Better Population Health; and C) Lower Cost;
3. When Behavioral Health resources are deployed and managed properly, they can help eliminate health care disparities and identify problems with social determinants of health across populations and geographies.
